Grace Mary Manor Case Management
Grace Mary Manor provides permanent housing to 84 men and women who are overcoming chronic homelessness. To assist these men and women with their transition from homelessness to housing, Grace Mary Manor is pleased to employ three full-time case managers and one full-time services coordinator. The Housing Authority of the County of Salt Lake, Volunteers of America, and The Road are each responsible for one case manager position at Grace Mary Manor. Case Managers provide residents with an on-site resource for access to government assistance programs, mental and physical health resources, referrals to substance abuse programs, and counseling to increase self-sufficiency. The Grace Mary Manor Services Coordinator provides on-site supervision to the case managers in addition to coordinating services from additional supportive service providers. The Services Coordinator also works to involve community groups and individuals in projects that will benefit the residents of Grace Mary Manor.
The Housing Authority of the County of Salt Lake is able to provide on site supportive services free of charge to residents of Grace Mary Manor through generous support from the Critical Needs Housing program administered by the State of Utah and Salt Lake County General Funds.
